Hotel Information: The FPA room rates available until the cutoff date of September 20, 2013 or until the room block is full.  Rates are available 3 days before and after the conference dates based upon availability at the FPA rate if available.  All rates (except those for the villas are based upon single/double occupancy. There are no resort fees.  Villa rates are based on up to four adults.  Additional adults are $15.00 each per night.  Reservations cancelled within 72 hours of arrival will be charged one night's room and tax (including no shows).

Check-in time is 3:00pm. Checkout time is 11:00am. Complimentary self parking is available.  Valet parking is optional at $12 plus tax per night.  Use of the 3,500 square foot fitness center is complimentary.  There is also complimentary scheduled shuttle transportation to all four Walt Disney World® theme parks as well as Downtown Disney in the evening for dining and entertainment options and to Orlando Premium Outlet.

Policy on Industry Support: The FPA adheres to the US Food and Drug Administrator's policy on industry supported educational activities. The FDA policy requires, among other things, that we conduct the educational program supported by any corporate educational grants independently and without control by the grantor of the program's planning, content or execution. Furthermore, the programs must be free of commercial bias for or against any product.

Cancellation Policy: If cancellation is made in writing 30 days prior to the program a refund will be made less a $10 service charge. If cancellation is made 29-10 days prior to the program, a refund will be made, less a $50 service charge. Cancellation requested in writing after that date will result in credit for another FPA CE program or service valid for one year.

Tax Deduction: The expense of continuing education, when taken to maintain and improve professional skills is tax deductible. Please contact your accountant for complete details.

Conference Handouts: The speaker handouts will be posted on our website, www.pharmview.com, after September 27, 2013 for you to print and bring with you to the meeting. Printed books of the handouts are not included in the full or daily registration fees but are available for you to purchase.

Registering by Mail?: If you wish to register for the FPA Midyear Clinical Conference by mail you may simply download and complete the registration form that we will make available on this site. Make sure that you note in the memo field on your checks that it is for the Midyear Clinical Conference. You can mail your registration form to the FPA office at 610 North Adams Street, Tallahassee, Florida 32301.


The American Pharmacists Association is excited to announce 15 new special offerings of the Pharmacy-Based Immunization Delivery certificate training program in 2013. Supported by an educational grant from Merck, APhA will conduct these special programs for a nominal fee of $99 to participate and should help qualify Florida licensed pharmacists for their immunization registry. 
 
The American Pharmacists Association is also pleased to announce four upcoming offerings of the The Pharmacist & Patient-Centered Diabetes Care certificate training program.. Through generous support from Novo Nordisk, APhA will conduct these special programs for a nominal participation fee of $99.
 
These programs are offered in cooperation with the Florida Pharmacy Association in the following locations in Florida:
  • October 22, 2013 - Orlando (Diabetes)
  • October 23, 2013 - Orlando (Immunization)
  • December 9, 2013 - Sarasota (Immunization)

Seating is limited so you may need to register as soon as possible!

The Florida Pharmacy Association is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. Up to 15 contact hours are available upon successful completion of attendance and evaluation forms. Universal Activity Numbers (UAN) will be available in the conference brochure. Statements of continuing education will be available for you to download and print within six weeks. This is an ACPE application based activity. Consultant number pending.
     


The Florida Pharmacy Association is also a Florida Department of Health Approved provider of continuing education and will be reporting to CE Broker.
